A Fund Tracker Slip includes your name & contact information, your Team Leaders' name, and the Donor's name & contact information. (First/Last Name, Full Address, Phone, Preferred E-mail). This contact information is used to mail a prompt “thank you letter” (for the donor's tax deduction/record purposes) and information about the upcoming HD Walk. HDSA-OC also keeps track of your individual and Walk Team collection amounts. Top individual fundraisers and top teams are recognized at the HD Walk.
